Assembled

Fourteen pages, just as they arrived. A made-up referral, in fax order.

Receive

The whole stack moves onto the lid. Fourteen pages received in this example.

Read every page

Each page is straightened and read. The reading bar follows the size of the page. The dark scan stays dark; the cut page is read from its top fragment, and the noise band beneath it gives nothing.

Account for every page

Every page goes to exactly one column. In this example: 10 Read, 1 Unreadable, 3 No clinical content, 14 received. The total completes only when the last page lands.

Build the timeline

Eight events, nine citation lines. Every line ends on a Read page. The injury event cites both accounts; it does not settle the date.

Show contradictions

Two accounts, kept side by side. Pages 3 and 10 lift equally. The bracket gives no verdict. All fourteen pages remain accounted for.

Review and sign

Page ledger, Timeline and Contradictions are generated by the appliance. A named reviewer signs the exact pages and sheets reviewed.

Export

Every page rejoins in fax order with its label. The signed packet moves toward the EMR folder. Illustrative order; the real export is a PDF.

Eight events. Nine citations.

Dates are invented. The injury event is ordered by its earlier reported date; both accounts stay visible.
